I believe I can reproduce it fairly consistently, but it takes a few tries to find the right spot. It's not like a pixel-width area though it's fairly large because it has happened to me three times by accident, one of them while I was aware of the issue and trying to mitigate it.
In the ledge in Blackburrow where the 2 Giant Snakes spawn, if you are very close to the wall (facing the ramp) and heal yourself, you will get several gnolls (none of the nearby ones) emoting their combat starts at you. Within 10-20 seconds, a giant train from deeper in the zone arrives and all of them immediately aggro you.
I have never seen anyone leading the train, and the emotes happen almost directly after the heal lands every time it happens. Several waves of the train arrive and it ends up being 10-20 mobs in most cases.
