But a guildmate of mine just did the turn-in of coldain heads to Bvellos, she got no faction hits at all. It's more broken than before looks like.

can you explain further? I do this quest regularly and the faction hits do exist. Look me up in game and we can talk about it. I have 3 characters on Kael faction. Are you referring to the turning in 4 heads at a time? Just only turn in 1 head to be safe.

The faction hits should be +40 Kromzek, +10 Kromrif, +10 KT, -10 CoV

It seems what the problem is in relation to charming Bvellos and trying to do turn in while being in the KOS faction range. Typically a charmed pet would take it to Indifferently for the non-charmer? So in theory it should still allow you to turn in the heads no? Apparently this was not the case.

Charmed npcs do not process faction gains

not sure whether the quest has been fixed in terms of turning in multiple heads at once, but I did it a few days ago (turning in one at a time) and it worked fine.
