You should be giving your confession documents to the first two NPCs once they agree to sign it, then tell the investigator to summon the guard and they'll be escorted away. (Hold onto the signed confessions til the very end.)
You should not have a second unsigned confession document on you at any point.
Turn ins should look like this:
> Turn in Shilster's cracked corrupt guard helm in to Vegalys, receive Investigator's Briefing
> Turn in briefing to investigator
> ask for 1 confession document
> Convince first NPC to sign document, then give the NPC the unsigned confession document to get Riley's Confession (keep that, tell investigator to summon a guard to take Riley away)
> ask for 1 confession document
> find second NPC and convince them, give the NPC the unsigned confession document to get Willie's Confession (keep that, tell investigator to summon a guard to take Willie away)
> ask for 1 confession document
> find Donally and 'convince' him, give him the unsigned confession document and he'll attack. The investigator will not assist him against you at this point (but he will attack anyone else who attacks Donally). Get his head.
> Turn in his head and the two confessions to the investigator
(As advised in the quest, ask for an extra confession document after the investigator respawns)
