The dread widow in the dreadlands should drop a drachnid thyxl 100% of the time.

I can confirm killing her and having it not drop.

I remember this dropping every kill as well, although mightve just been every time I killed it due to luck and high % ratio

yes from my personal experience it has always dropped, i will add in some quotes from the drachnid thyxl link:

"The drop may not be rare (since it's a guaranteed drop) but the spawn is, which is why the item is listed as rare."

"I have killed the Dread Widow atleast 9 times in the past 2 months and in one sitting killed her 3 times. She as ALWAYS dropped the ER3 Item. It appears to be a guarantee drop off her. Definately NOT Rare. "

Of course it is difficult to rebut a 75% claim, especially because most people who write, do so to tell their story of killing the mob at one time. However if you read them one by one, you will not find a single instance of someone claiming to have killed her and having it not drop. That is, everyone who has written about killing her has also written that they got the thyxl.

I've killed her three times. She has dropped it once. I didn't see it in her hand when she had it, though.
