I made these 2 changes (as well as moving the game out of Program Files which you've already done) to resolve this issue
Run the game in Windows XP Service Pack 3 Compatability Mode.
Run the game as administrator.
Good luck!

Necro this thread, I continue to have this problem. I've tried every compatibility mode option. I've tried reinstalling Microsoft Visual C++ Redistributable packages. Is there any way to prevent this?? Always happens when camping out.

Well then its a question do u want to crash in EQ all the time or do u want to take the chance and fix it:D You can also try search Voodo or whats it was called again in Search p99 and u will see that more players have recommended this or you can do self research about the program in google ?.
When uhave installed it right u will get a dgvoodo watermark at the right botton of EQ screen. U can take this away in settings of the program. U never have to start the program again only run EQ as normal after getting it right the first time and no more crashes :D.
I would also recommend the GMs here to maby make a post or test this one for this is the solution for those who crashes. Atleast they who have gotten the "reset Device" fail / alt enter - zone crash - change char crash

I downloaded the latest stable version at the link after doing a bit of research regarding its authenticity (seems legit from what I can tell) however I really have no idea what Im looking at here or how to actually use the program to stop these crashes.
Any input on this?

Copy from Mcoy from another post:
Read the html when you extract the zip. Basically, you copy the DLLs to your EQ folder and that's it. All I did was extract it to my EQ folder and edit the conf file so it doesn't include the watermark.
So
1: Copy the DLL files to EQ folder
2: Start the game
3: if u dont get dgvoodo watermark at end of screen i think u need to ADD your EQ folder in the program. I can not remember if i did this

Okay, I haven't had a crash since installing dgVoodoo and running in compatibility mode for windows xp service pack 2 with admin privileges.
The program does look legit and seems to help patch old games to run in newer computers for windows by providing certain graphics DLLs.
I downloaded the zip from here (current version is v2.72):
http://dege.freeweb.hu/dgVoodoo2/dgVoodoo2/
Then unzip that, and follow the instructions in the in QuickGuide.html.
I ended up moving all 7 .DLL files into my project 1999 game folder and haven't had a crash after camping since.
from the extracted 3Dfx\x86 folder:
Glide.dll
Glide2x.dll
Glide3x.dll
from the extracted MS\x86 folder:
DDraw.dll
D3DImm.dll
D3D8.dll
D3D9.dll
Then in the project 1999 game folder, right click eqgame.exe, go to the compatibility tab, and make sure you have windows xp (service pack 2) and "Run this program as administrator" checked.

Worked like a charm, thank you for the detailed explaination! Any info on editing the config and removing the watermark?

Run dgvoodoocpl.exe, point it to your EQ installation (where you copied the DLLs and conf file) and then uncheck the watermark under the directx tab:

Interesting, I had both watermark checkboxes unticked when I did the above .dll copies but the watermark was there for the first while. It has since disappeared.
Thanks again for the answers, this has been huge for me!
I think it was Savok who told me about it - he deserves all the credit.

It happens often, but because it's 1 guy and the author has to report each new version it takes time to get each one whitelisted with every AV vendor.
You don't even have to use the exe if you don't want - just manually edit the conf file to change the options.

Does this not work with wineq2? When I attempt to launch, I get the message "Please remove the file D:\\Sony\EverQuest\D3D9.dll"

I had this same issue, but I run it as Admin, and switched the compatibility to Windows XP service pack 3 ,and it works fine now

I added these items to my folder, and I am now getting this weird error:
https://ibb.co/NFY5hxH
Any idea on how to fix? Sorry I'm not super tech savvy.
Sadly the dgvoodoo stuff didn't fix my "crash" issues, though perhaps mine is a bit different. My game doesn't crash it will just stop responding on zoning randomly or when changing characters. Before I changed it to admin priv/compatibility with SP3 the freeze would require me to reboot since I couldn't minimize the window to close it via task manager.
Interesting side note, when I did try all these DLL's it made it so going full screen and back to windowed no longer fixed the blurry text/UI.
