P99 Green Crashing - Device Failed

Oddly enough, randomly when I attack a mob (1 out of four times or so) p99 freezes and crashes. I think this is related to fullscreen/windowed issues others have experienced in the past, but I could be wrong. This happened rarely before, but not it happens all the time. I do have a dual monitor setup if that matters. I get crashes in windowed mode and fullscreen.

Very frustrating, I got my little SK to lvl 9 and then this started happening. Solutions I've tried:

- I entered eqclient and manually changed my resolution
- Reduced max FPS to 60

Please help -- I was having a ton of fun :(

Should also add I have tried fresh install.

Let's see - a level 9 SK. You can summon a pet now. Are pet hits (or other combat related text) going to a separate window that's perhaps set to an invalid size?
