Me and a friend have both had screwed up hand in returns from a failed MQ in Kael for some Forest Stalkers. We think there was a faction pull in between so death hasn't even reset the issue. Wait for a quake/ reset before you hand in.

I don't remember mobs keeping quest items after death on classic. Did it really used to be that bad back in 99 as well? Guess I never ran into it because people weren't intentionally griefing armor turn in MQs.

Probably someone with too much time on their hands sabotaging MQs. Start with ones that dont matter and make sure you clear them for the ones that do.
