Please bear with me. I know this may sound like an incredibly dumb question...but, how true to the 1999 game are we talking here? I am going to return to the game after a looooong hiatus and the thought of playing the game I started on is appealing. Is it the original Death penalty? Long, long Cr's are not the problem for me...getting someone to help get my body back after I fall into the Hole may be! Is the UI customizable? Is the engine updated or are the original graphics (i.e. Box heads) the way to roll? I have no problem playing the game with nothing but original spec's, just curious before I go home and load the game up.

Yes pretty much to all answers. It's old-old-school. Original death penalty. Corpse Runs at all levels (YMMV). All Original graphics, you can't use the new models (you CAN update the textures to higher res though, which makes it looks nice!). The UI is definitely customizable (go check in Technical Discussion for people doing this).

But some things just can't be taken back to original because of the client itself (which is based on Titanium or SoF) - such as original wolf models or original skele models don't work all to well here, so you'll see the new ones in game. Boats aren't functioning yet (they will be soon thanks to Mixxit!), so for now Translocaters are being used.

But most of the original flavor is here!

It's pretty close. The OLD UI isn't enforced, in fact, you can use the newer customizable ones. The Hole isn't even open yet, so no worries there. Death is painful on the exp bar. Luclin models are disabled.

There are several minor things that aren't 100% classic, sure, but for the most part, you are getting the most 1999-ish experience of EQ that you can get.
