They mostly spawn between the Mill and the ruins, but can be a bit further North. That list of mobs are the ones to target. If you have track just kill the ones to the top of track and work down. Soon you will get a mob that I've figured is definitely on the cycle (sabretooth cat, sarnak youth, sarnak flunkie, iksar brigand). Once you see one of these, you have hit on the cycle. Kill it and watch for the thing that pops about 10 seconds or so after it. That's the next mob on the cycle. It can be anything on that list. If you lose the cycle, revert back to killing everything on that list at the top of track.
The addendum to this is that it *seems* there can be more than one cycle going at once, as I've seen two PHs up at once. This was true on live too. It may be there can be several going, and if you have the power to do it, kill every single mob on that list in the area and keep killing. That may be the best way to spawn the courier. In about 8 hours of hellish green killing and tracking, I killed upwards of 20 or 30 couriers, so it works.
A guy who posted originally during Live said there will only ever be one of those cycle mobs at a time on track. Others disputed this. I can tell you that on p99 there can be countless numbers of them, and I'm pretty sure only a few are actually cycle mobs. The only way to find out which is which is to kill and watch what pops to replace it.
It's a super rare drop, on a rare spawn (someone on live killed 1000 couriers and got two rings), so be prepared for a possibly long and frustrating camp. Goblins will also hammer the courier if they see it, and it won't survive more than 2 nukes so you better be quick. Also, the idea that the courier with a ring will have the light glow on him from the ring is not true in my experience. So... just keep killing.
