Realistically, for every player item loot would bring, it would probably cause 5 or more to leave early or never start. That said....
Since everyone logs into red PvP flagged, what if turning in the book permanently flagged you for item loot. Non item loot flagged players can kill as normal, but never get to loot items. Item loot flagged players can kill players as normal, but can loot items from other item loot flagged players. Throw in a 10% xp bonus for players flagged for item loot so there is a kind of risk vs. reward feel to it.
As far as identification and shit talk / bragging rights, just have it add a <HC> or something after the name...

I am all for enhancing the experience of EQ PvP with more options, just like we had in 1999 with 3 PvP servers, but flat asking for item loot when it was always the least popular option is dumb. I played RZ and SZ on live, and neither of those servers kept the population levels that VZ and TZ maintained.
