"Duplicate Lore Items Deleted" message upon death
 

I am unsure about this one. I died and got a random "duplicate lore items deleted" message. It could be that the game was copying my keys to skyshrine and cobalt scar to my naked person and registered it as two lore items existing on one character? At any rate, it seems indicative of some sort of bug or transcription error. Probably a minor one.

Additionally, and separately but you can see it in my screenshot, what happened here was that I was trying to swim up the waterfall in Siren's Grotto to get to Western Wastes; my guildies remembered this being the route and that it was possible to go up the waterfall, but I was unable to go up it. I don't know if they're remembering wrong or if there is a bug preventing people from swimming up the waterfall, but now it's out there for you to do with as you please.

Should be resolved now.
