Things falling into the earth
 

My pet and/or the mobs he was fighting sometimes fall into the earth, causing problems ranging from that mob being stuck and my pet having to be killed, to massive trains killing me and anything in the train's path.

This is happening all over the place. This was a problem before and it got fixed.. Whatever was done to fix it previously appears to have been undone during a recent patch.
