Some additional info:

I still encounter this frequently, and in LoIO this time. I've noticed a clue as to what may be causing it, and it requires unfortunate timing. The landscape geometry. The triangular/pyramid-esque bits of ground that make the landscape more believable seems to cause this. Should your target AE hit the quad at seemingly the precise moment it is on the top of the landscape geometry, the spell will consume mana and not hit. It seems as though the client is deciding between the mobs being in LOS or not, and doesn't decide in time whether to continue or interrupt the spell so the client kind-of does a bit of both hehe.

I have been able to avoid encountering this problem entirely while quading on completely flat landscapes. The landscape geometry seems to certainly be a factor in this bug.
