Not sure if anyone else dug up the update info, but for GM's notes on applying Velious updates -
Our wiki says it was changed from the 6/13 version to the 16/19 version "between Sept and Nov 2001" and links to this LoS news page with the old version -

In fact, it appears that it was actually changed between 10/6/2001 and 10/13/2001.

I don't have links to the old patch notes handy, but I'm sure there is only 1 patch in between.

Furthermore, I went through all the patches between LoS's update (8/29/01) up through October, there is no mention in the previous patches of any update. The only logical option is that it went in with the 10/8/01 patch.
EDIT2 - 10/8/01 patch is looking like they added several weapons as part of the melee revamp and went ahead and made a bunch of knight/monk weapon additions and tweaks (rather than doing it in the future). Examples - Gharn's changing, adding turnin mob for Primal/Priceless Velium Fist Wraps and Knight's Swords, Fist of Lightning being added -
