I've gotten back to back to back impaler swordfish as well as two fierce impalers within 10 minutes before. It might favor a specific spawnpoint (much like shellara ebbhunter) but can spawn instead of any lancers.

Odd. I've been soloing/duoing on that first floor for weeks and it has always been those same two spawn points I mentioned, and never had a fierce impaler/impaler pop more than once in the same spawn cycle.
