It received data from the zoneserver regarding combat, etc., but had no clue who the source was, so it displays the unseen hand message. You likely would not have seen anything from that person in /say, /auc, /ooc, /em, /shout etc since those communications are handled by the zoneserver and not the worldserver. Tells are handled at a global level and thus work properly, same goes for /who all.
