Lieutenant Milven was added with the Nov 2002 SS revamp as well.
The quests for Giant Toes as well as
Giant Warrior Helms should be on the two entrance sentries from WL side.
I believe they took Storm Giant Toes and Hill Giant Toes(bug here not sure when it was fixed to Frost Giant Toes).

Its Storm Giant Toes, not Frost Giant Toes. And the turn in MoBs are the same for the helmets for faction, the two guards at the WL entrance of SS.
