I'd start with the one that seems to trip the most people up;
you need to make sure your in-game resolution is set to what your monitor is.

What's your native desktop resolution?
Open up eqclient.ini and change the windowed/fullscreen resolution to your native desktop resolution.
If that doesn't work, try running the game in compatability mode and 640 x 480 resolution.
If that doesn't work press alt + enter some time before the server select screen.
