This is the Coldain head quest. When you turn in a Coldain head, you get some faction and a few plat.
Bvellos is not recognizing when there is more than 1 head in the trade window. If you submit 4 heads at a time, you will only get credit for 1 head. No other quest I know works this way and there are no mentions of this on Allakhazam.
If you submit 4 Coldain Heads at a time, you should get 4 rewards. I just wasted 28 heads turning in 35 because of this. I was only rewarded for one head from each turn-in.
