100% can say if you're playing 4:3 on a tiny monitor you're missing out. Plus, I have the option to play 4:3 on this monitor and it looks terrible.

"it looks terrible" Um, did you set your GPUs output resolution to 4:3? Yes that would look terrible. No, I did not go out and buy a "tiny 4:3 monitor" just to relive the glory days... I use a 34" 3440x1440 monitor.
Of course the first thing I did coming back to P99 after 6 years was to set my resolution to 3440x1440 full screen. And then I went into the warrens with my Ogre SK and could barely play because I could not see any of the kobolds attacking me in first person. 3rd person it is ok, but squarer is still better. Even going to normal widescren 16:9 helps, but I find closer to 4:3 is best.
By the numbers:
at 1920x1440 viewport, I am using 350% more pixels than the average 1024x768 resolution this game was designed for. So... the graphics are hardly impaired.
at 1920x1440 viewport, I have a viewport 79% larger than someone playing at 3440x1440 (same horizontally, 79% larger vertically)
I have 1520x1440 pixels left on my screen to do useful things with. UI elements, p99 wiki, other browswer tabs, etc.
It is not the most cinematic experience, but it is the most practical and playable.

Assuming you have a 3440x1440 monitor, try this:
eqclient.ini:
change the resolutions in this file to 2200x1300 (this is close, play with this to make it fit well with your window bar and task bar)
launch the game and move the window to oneside side so you can do something useful on the other side.
in game do /viewport 200 0 1800 1300. You now have 200 pixels of black bars on each side to store UI elements you don't need quick access to. I play the same game you do, I know you have to click things and move things around, you can pick and choose which elements are over display or to the side.
