Personally I'm not impressed with "first" type messages as they generally just reward people for racing through content via foreknowledge, exploits, luck, or just sacrificing RL. I'm not saying don't have them, I'm sure others would enjoy it, but I hope people suggest non-first achievements as well.
That said, if you add non-trivial custom quests/items, a first message would be neat for those, since it isn't just a paint-by-number zergercise and would really be a first achievement. Only on Blue I'd imagine, but still...
As an NTTR player it would be neat to have achievements for not dying, though that doesn't seem as simple as log parsing without some stored state. E.g. for getting to 20, 30, etc. without dying.
Longest stint without eating/drinking.
Most HPs healed in one cast (0% CH ftw) caster/castee.
Most money spent at a vendor.
Largest single sum destroyed.
Most items given away (nothing in return).
Most misses in a row.
Most consecutive mobs killed via Bash/Slam only.
Most consecutive fizzles.
and of course
Highest gems score.
