Normalization of Deviance

Coined during the apollo up to the challenger disaster.

O-rings were leaking, it was too cold for the o-rings to function properly and the manufacturer that brought that up got kicked out of the meetings.

That is the Normalization of Deviance.

You get away with it so many times that you continue to deviate from the norm.

The root cause of it is, Poor Management, Mind you a fish rots from the head.
