I believe the hair clipping through on Barbarian, Dark Elf, Wood Elf, and Human females may have been that way during Velious on live.
I took screenshots this week on the
Trilogy Character Select. They show each race in their custom helms and those four female races appear like on P99. I admit, this is not an entirely credible source. The proof-of-concept Trilogy server I used, by Zaela, could be missing some instructions for the client to hide the hair.
However, I also found a screenshot I took on live, prior to Luclin, which shows hair clipping too:
