I suggest it was the entrance to the goblin city, long ago.
When the children of Zek were punished for attempting to overthrow the gods tectonic activity destroyed the city, subsuming most of it, leaving the area scattered ruins and underground cities.
Or maybe it was a similar cataclysm caused by the imprisonment of Lord Nagafen.
Whatever the disaster that sank the castle, riddling it with tunnels and lava, the goblins then adapted into lava goblins.
