Crushbone/Deathfist pads were both named the same until the Dec 4th 2000 patch apparently. Just "Legionnaire Shoulderpads".
You had to be sure of which ones you had in your inv, Deathfist ones wouldn't work with CB quests and vice versa.

Since the Crushbone "Legionnaire Shoulderpads" were renamed to "Crushbone Shoulderpads", they don't work in the Banded Orc Vest quest.

This is very important to know about Legionnaire
shoulderpads. You don't want to get the ones from
death fist to give to the guy in Kaladim. I found
this out the hard way that they don't work. Also
watch out for people who will sell you pads from
death fist... You can't tell it's from there but
if they did sell you counterfit pads mark them
well. As far as death fist pads go they should
mark them as being from there and I don't know what
they are for.

Banded Orc Vest quest seems to have accepted either pad before patch, but afterwards only Deathfist.
