Pearl Kedge Totem should mainhand usable
Referencing the wayback machine and old allakhazam's (plus EQLizer), Pearl Kedge Totem should be mainhand usable AND non-lore. I'm not sure when the PKT was changed to secondary only and lore. Further research is necessary.

They didnt list primary or secondary on the item, how can you tell which it can go in?
The listing of a slot was an addition later ( unsure when exactly ) and they added lore items all the time. Lore items were not part of classic, then a bunch of items became pending lore as in they were not lore to start and likely could have made a screenshot prior to that new tag.
