The Penance Quest

I have done this quest twice now to no avail.

I harvested the [Bone Fragments] with the [Hammer of Exoneration] like you do. I grinded those with the [Grotesque Mortar and Pestle] and put 7 piles of [Powdered Bone] in the bag with a [Flask of Bloodwater] and combined them. The first time I did this, I did not include the book. So I started over, did all the steps again and gave the stuff (bag and book) to Zand (I'm an Ikky Shaman) and nothing. No dialog, no nothing. My goods just disappeared and nothing. Is it bugged? Am I doing something wrong?
