EMU Customization

Checking out some of the Servers I see Customization mentioned often, though I understand the concept, what does it really entail? For example a custom NPC, does this mean a certain list of items are on that NPC that you can buy?

I've also seen custom spells mentioned and custom quest. I'm kind of curious what all this means.
