I can't pinpoint when exactly, but I've been getting some lag sometimes. it may have been an NVidia update that's causing it. does anyone know how to resolve? my PC is pretty powerful, so I shouldn't be receiving these kinds of issues.

What kind of lag?
Mouse look, stutter steps, clouds, mobs lagging, stab lag , cast lag?

mostly for mouse-look, but also using wsad - it's like a slight lag when there's a handful of PC/NPC. I was messing with max frames and it makes no difference.

I was having same issue, and I think I may have finally found the cause. Turns out it was Gina, specifically the auto archive log feature I had turned on recently. Since turning it off and manually clearing my log file I haven't had any lag issues.
Other things I tried when I was looking into possible causes that you can try: running EQ off of a ramdisk/SSD/flash drive running EQ with winEQ or off of one core by putting "C:\Windows\System32\cmd.exe /C start /affinity 1 eqgame.exe patchme" without the quotes in the target line of your EQ short cut or by using the DuxaUI installer to make a shortcut for you running EQ in compatibility mode set to xp sp2 or sp3
