Default Track not showing correct PC/NPC locations

I've noticed that occasionally track will point me in the wrong direction of whatever I'm tracking. Last night, I was tracking a player that I could see in front of me, running directly toward me; according to track, the player was behind me. It corrected itself when the player got closer. It seems to be an issue of track not updating properly, as I have also experienced this issue with roaming NPCs, but not static ones.

Did this happen prior to the latest patch?

This has been broken for a long time. Track usually works but sometimes devastatingly doesn't.

I've noticed it since I started in April, but it happened three times yesterday, so that was my impetus to finally post about it.
