Procs Per Minute

Hey guys,
I'm wondering about procs per minute and weapon swapping. Does a proc 'counter' reset with a weapon swap? For instance, let's say that a Polished Granite Tomahawk has 1 proc per minute on average. I activate Berserker Strength within my first 10 seconds of swinging and switch to my Whip of Strangulation, which also (for the sake of discussion) has 1 ppm. Does the activation of the PGT factor into the proccing of my whip?

Would it be a viable setup to constantly be swapping obsidian shards out with weapons like the PGT and the Iksar Berserker Club for offensive/defensive proccing madness, or am I overestimating how frequently I'm going to be proccing as a halfling warrior with ~150 Dex unbuffed?

Also am I being paranoid or does it seem like accuracy goes down immediately after you swap weapons in combat?

Basically trying to make playing a Warrior more fun haha, let me know what you think.
