New very expensive equipment and doctors prescribing them too frequently, such as MRI's, using surgery machines, etc. Mainly because we demand them to. The new requirements of Obamacare add a small amount to this as well, but its no where near all of the other factors. When it comes down to it, costs continue to rise and all of us demand the best possible (meaning most expensive) care available in the modern world. Someone has to pay for that.
