Lesser Spirit not spawning when Capt Surestout killed in Ocean of Tears

Was killing the pirates for FV faction and wanted get the tiny gem as part of the shaman epic, killed Capt Surestout at least ten times and lesser spirit only spawned once. Everything I have read on the shaman epic indicates the lesser spirit should always spawn?
