Purified Crystal Spear combine for Spirit Aid Quest broken (deletes hammer)

There is a no fail combine for this quest:

Soulforge Hammer + Crystallized Shadow Spear in a Forge

It is supposed to create a Purified Crystal Spear, a Soul Orb, and a returned Soulforge Hammer.

A very strange thing happened instead.

I got a message: You cannot combine these items in this container type!

The items remained in the forge.

When I closed the forge, the hammer was deleted. I believe the spear was returned, but I may have had another spear.
