To this day, I'm convinced that "The Painting" everyone is obsessed with in RocknRolla but you never see, is that very image of these siamese dicks, gently resting against one another much like a snuggling family of otters.

So to answer your question, perhaps there is no sword fighting at all, just a nestling together of smooth and occasionally pulsating skin . Eventually, when release is desired, you do a Rocks-Paper-Scissors, and the loser grabs a hold of both dicks simultaneously and attempts to get both off at the same time. Tradition states that as this feat is achieved, both must cry out in unison, "BOOK NOT UP!".
