You should see a quest NPC as a portal for you to interact with a quest inventory. Killing it doesn't remove or reset the inventory, it just closes the portal until respawn. A full server restart (NOT a quake, but a full restart) resets everything like its a full reboot.
If a quest needs A/B/C/D to complete, someone hands in C, and someone else hands in A/B/C/D, the following happens: player #2 gets the item reward for handing in A/B/D, the only items needed. Player 2 then also hands in C, which wasn't needed, once more stacking the NPC with item C.
This is why you should, if you are able to do so (i.e. if you are completing a quest yourself), hand in items one at a time. If the NPC was messed up, you 'fix' the NPC this way by completing the item, and you get to keep the item that someone else already handed in.
This stuff is often used on velious quest turnins because some gems are cheap, so people try to fish for free items by turning in just the gems.

Thanks that all makes sense. It also explains why staff do not litigate MQ misfires and I don't blame them.
