A similar thing happens to me when I play, but it seems something different triggers it. It might be hard to follow my explanation, but here goes.
In certain zones, when I look at certain things, like a hut in EC or the middle of the lake in Cab, or pretty much any ruins areas outside of cab my game freezes. The UI goes away and the image on the screen is frozen. I can alt+enter to minimize the screen and keep playing, but even in the windowed mode if I look at those same areas I freeze and have to alt+enter again. If I scroll up or down while the screen is frozen, when I minimize and come back into full screen I can move again, but I have to be looking straight up or down until I am far enough away from the trouble area.
This didn't used to happen, but I took a break from the game and when I came back after Kunark it happens in almost every zone. I can get by, but it is a huge pain in the ass, and also makes for an interesting gameplay, having to avoid looking at certain areas in a zone. Weird.

When my screen is locked, or I change resolution, the same black screen appears. I have to blindly /QUIT to go back to main login. Windowed mode, always. It is a video card issue (not a P99 issue).
