Gfay was always a good starting place in everquest, but on p99 its excellent because people tend to congregate there. You should definitely do orc hill starting at 4 and by 6-7 go into crushbone for a while. Sell the belts/pads or turn them in for exp as others have said.
Starting in the low teens you have tons of good options, its mostly personal preference. You can do the karanas, unrest, blackburrow, oasis, upper guk, ec, befallen, the list goes on. Oasis always has people in it and is centrally located with a bind point not far away. Lots of people do unrest but I never go there personally, if you die getting your corpse can be awful. Blackburrow and the karanas are never packed but you're rarely alone. If you are new to the p99 or eq in general, stick to the well traveled areas and make some friends. Join a leveling guild, its a great way to meet new players and when you hit 46 in a few months you can find a guild that does more of what you want to do

First time playing any everquest game.
Definitely looking forward to joining a guild, things are a bit lonely at the moment. Thanks for listing those for me I'll have a look at what looks good to me!

If you stick with this game, you should put a lot of thought into which guild you join once you hit 46 because you're throwing your lot in with a group of people in order to do high end encounters and build up the chance to get the best loots possible. (46 is the minimum level for the planes and is a common cutoff for joining many medium-high end encounters.) You should put very little thought, however, into joining a leveling guild. They come and go every 3-6 months. Just join one, it will make your life a lot easier. You can always leave by targeting yourself and typing /guildremove

I've literally walked from East Commons to Innothule, to Rathe, and up into the Karanas, just taking in the sights and killing any blue con or lesser that dares cross my path. I did the same thing in Kunark. Everquest is alive and has gravity in a way that no other MMO really does, at least to me.

No. You go to 99% as a buffer though from what I remember of my time on live. As you get higher though you'll come into contact with rezzers (clerics, pallies, and necros) or at least be able to "buy" their services. Rez in EQ is more important for its XP return than it is for saving you traveling (actually you may have to be in the same zone to be eligible for res? not sure).
