I camped the south ro one on my rogue at 41. I had mqd my epic though(and you could wield it before 46 at the time) and had no issues slaying him. I'd think untwinked at 45 is very possible. Not sure why all these posts are suggesting 50+.
Because the ph in oot is a cyclops with a shit load of hp compared to other cyclops.

I would just bite the bullet and MQ one. I did the Sro camp for 2 weeks and had it taken 4 times by one guy alone not to mention the other campers. unless you are a druid ranger or bard you cant win the race in sro. This is just one of those easy money camps that brings out the level 60s every time and they have it down to a science. Unless by some miracle it spawns on top of you, they will tag it before you have a chance. You are there because you need the speed boost they already have it. It's just that simple.
