Mobs warping and pathing through walls

We just had an imp protector at efreeti. Root broke and he ran through the wall and made it all the way to entrance, this has happened tonight in solb HS and chardok

Similar thing happened today in Seb @ king exit room. Group engaged a reaver that got pulled in (King PH). It immediately poofed through a wall and summoned a couple of us in with him.
