Wrong Spell Icons are Displayed

Most of my spell icons are randomly swapped. Like Spirit of Wolf will be the damage shield icon or healing spells will be the Skin like Wood icon for example. It was like this on default install and trying to install a different gem or UI will just break the UI.

HOW DO I FIX?

You can try re-applying the latest patch or just the uifiles folder from it. All the UI files are in every patch

I tried that a couple of times and still no luck.

Are you using the Velious UI?

It doesn't matter if it's Velious, default, deafault_old, all 3 the icons are randomly swapped
