You did install Titanium right? Not like Trilogy or any other expansion?

It's the Windows Compatibility. Click on properties of the icon you're using to launch p1999 with and then compatibility. Make it to XP SP3. Check a box that says to run as admin.

Nope, installed Titanium... and sang the damn song the whole time which is still stuck in my head.

Installed CCleaner to see if HP Protecttools was installed, it's not, since that's been an issue in the past for some.

Is your EQ file directly off of C? If not, move it there. Use no spaces in the target line. You will need to adjust your shortcut, if you move the file.

so target line (shortcut) will be very similar to:

C:\eqp99\eqgame.exe patchme

Will add compatibility mode/admin note to FAQ, since I can't remember putting it in there already. Both absolutely need to be on to run EQ on pretty much every machine in existence right now.
