Game Mechanics: Lady Vox still not ae'ing correctly

On the most recent kill, Vox did not AE properly.

She was agro'd while the giants were being pulled out of the lair. She made it out of her lair as far as King room (maybe further, I zoned out).

Saurus's body freezes as the frost hits them.

Demetrium's body freezes as the frost hits them.
Kenantik's body freezes as the frost hits them.

Webelost's body freezes as the frost hits them.

After she reset and we finished clearing back and moved to the lair, she did not cast Frost Breath again.

She was killed in the lair.
Her head was not in the wall.

Out of curiousity, why would you kill Lady Vox after bugging her so she doesn't aoe instead of logging out and petitioning?

The GM response rate to petitions is approx .3%.

We did not intentionally bug her. She agro'd on the puller, then the puller FD, but agro had transferred to someone not in the raid who was near the lair. Amd then she was out of the lair.

We did not know she was bugged until we engaged her in the lair. But here is the appropriate bug report; full logs of the encounter have been sent via pm.
