I've had the same issue, on both Win 11 desktop and a Win 11 VM instance on MacOS, pretty sure its isolated to Win 11. Haven't found a fix for either, but someone mentioned maybe EQ playnice might solve it?

Last time it happened i just left the error window opened and moved it to my other monitor, and kept playing P99 just fine, but when i zone it crashes, so at least i know i have time to kind of get situated before the game closes...

Just wanted to update my experience. I changed my refresh rate down to 60mhz and was able to get it to launch. I have windows 11.
