Amontehepna in ToFS
After clearing the 4th floor for several hours, I didn't get Amontehepna to spawn. I did see the named Oozing Flesh (Maggot Infested Flesh) spawn 3 times.
It seems unlikely that the former would be this rare and the latter this common.

She spawns in a specific location after you kill her trigger. Did you look for her next to the mirror for next floor?

It's possible to go that long without Amontehepna. Can be a bitch sometimes.
FWIW I cleared that floor fully (Amon included) about 2 weeks back.

Amon deffo spawn. I spent like 9 months camping a pair of skinning knives. Took well over 30 Cara kills, ergo at least that number of Amons.
Run around spamming /target Amon (or even better /pet attack amon). Cycle behind the locked door too. Check he didn't fall into the tunnels under the level either. I think he can spawn even if Cara is up, but won't guarandamntee it. Good luck.

So, to spawn Cara there is a process. You need to wipe the area until Amon spawns. He can spawn in place of any of the trash mobs on that level.
Once you have killed Amon then Cara will instantly spawn in the room with two coffins just before the portal to the next level. Looking at the mirror Cara spawns sitting in the left coffin. Good luck!
