Disconnect issues when zoning

I am having issues with disconnecting when I change zones. It happens about 75% of the time. The load screen comes up then it kicks me back to the account login screen. Also, sometimes when I login to the server and hit "enter world" it will kick me back to the account login. If anyone else has this issue or knows of a fix it would be most appreciated.

Also, sometimes in certain zones everything is extremely laggy and choppy. In gfay for example, I cant go up the elevator to the city because i fall through the elevator every time.
