Ranger Issues
Currently no disciplines are available for Rangers when /disc is done in-game. I don't recall if hybrid disciplines were added with Kunark or Velious or in between sometime. I've been searching with the waybacktimemachine, but haven't found anything solid.
Evidence: (Help find some!) Storm Strength (53) and Chloroplast (55) are currently not able to be scribed as they show a lvl requirement of 255. Hybrid disciplines were added with Velious. Only pure melees received disciplines in Kunark.
http://www.eqvintage.com/links/melee-disciplines-2/ Jolt was not in at Kunark launch... it was added after aggro issues for rangers became apparent beyond level 50 as everyone began to level up.
