Im jealous, I cant figure out what is causing my issues besides the dll file.
the V49 patch dll file name is lower casem rename the file to capital

renaming dsetup.dll to DSETUP.dll
hard reboot

does NOT correct the issue for my vile p1999 macOS 10.14.6 (18G95) supplemental install

do we need to put back Nektulos files or something else?

On a MAC you CAN'T have the same file name whether it be dsetup.dll or DSETUP.dll STOPPPP GIVING BAD ADVICE. We are talking MAC not LINUX. If you are really on a mac try copying the same file to the same folder and having the same names, it will not work.

Try reinstalling to a clean one word drive. like C:/eqemu. instead of C:/Program Files. I did these two things and was having the same exact issue and thisnis what fixed it for me

to note When i renamed the file on first reinstall after trying to launch the game the first time It did not work for me, a reinstall might be manditory at this point if renaming the DSETUP.dll in your eq filder is not working for you. but too they may be more issues. its all a maybe at this point. seems to work for some right out of the box amd not for other. some work on rename. some bot. hard to tell

Renaming does nothing... the v49 dsetup.dll is what's causing wine to not open. The original DSETUP.DLL lets wine open. The v49 dsetup.dll will not allow wine to open whether it's capital or lowercase.
