This guy spawns by the hill giant area (#23 on eqatlas map) and has a roaming place holder "an orc priest". When killing all the orc priest in this area I noticed one was still on track and found that its under world inside a mountain as i was able to see through wall by positioning at the right angle. Ironically enough this problem is reported on live as well in later ala post. While he may be there on live he obviously wasn't for at least a short period originally.

I have a screenshot that I took Jan. 11th of a mob with this name underneath the Grounds of Fate in Qeynos. It was glitched and had a model of a generic human (the default model when things glitch) and was quite large and aggressive. I don't have online hosting so I can't post the shot, but I can mail it to someone if it'd be useful.

Thanks for the link. Let's see if it works. This should be a shot of the aforementioned NPC standing under the Qeynos arena.

EDIT: Note, also, that the location of this NPC on the way to the Qeynos evil guild, and its universal hostility, means it will likely accumulate newbie corpses around it.

Grazhak doesn't spawn near HGs on this server... no idea where he spawned on live cause I've never seen him.

Update: This NPC has despawned as of the time of this posting. Whether naturally, forcibly, or by being killed, I do not know.

not fixed, right now I see it on track under the world. Was wondering why ghazek the berzerker or its PH wasn't roaming now I found out why. for 2 hours.. looking.

looks like bugs from 2 years ago reverted on the last patch - (dispells being resisted, fungi tunic broke, this)
