About 2 patches ago there was a improved pathing update, and ever since the update the mobs have been doing strange things, here are some of the things i have noticed.
Since the patch, it seems that sometimes mobs want to take the shortest path to the person with agro...a straight line, sometimes this path is right through a wall...where before they would use the pathing nodes to get to you.
The problem with them pathing into the wall, it seems when they do that, sometimes there is no collision detection inside the wall for the floor, so the mob falls into the world, at some point the game must reset its position to a "good" position in the zone and then the mob finds his way back to you with a huge train.
Also when a mob paths into a wall, if there is any rooms/halls under or above the wall, the mob may instantly warp to the lower/higher room and start pathing from there, bringing all the mobs in that room and any along the way back with him. This happens in KC / OS / Charasis / Lguk, pretty much anywhere there is valid geometry under or over the position the mob falls into.
This also makes caster mobs slightly more annoying also, where before you would run behind a wall and wait for them to path around to get LOS, now sometimes they just path in a straight line into the wall and clip out the other side and start casting on you , or fall into the world and train like I said above.
The only way to prevent it, is to hold the mobs hand as you walk it back to the group and make sure it does not try to clip into a wall, and sometimes this don't even work, I'll hit a mob and turn around to run and nothing is behind me, it just falls into the floor, and then 1 min later a huge train shows up.
TL DR = Mobs pathing into wall, fall into world, bringing big train. Also can fall though floor or wall into the room below it or above it, bringing big train.
